I hereby apply to become a member of Musina Top Runners (MTR)/renew my membership with the club. I confirm that all the information provided on this application form is true and correct. I agree to abide by the rules and regulations of the club as laid down from time to time. I subject myself to the rules and regulations of Athletic South Africa (ASA). I am medically fit and understand that I participate/enter in any activities/events entirely at my own risk. I agree that MTR, its officials, sponsors, supporters or members shall not be liable for any accidents, injury, loss, damage, death or claim or expense that may arise in consequence of my participation in the club activities/events whether formal or informal and whether as official or competitor.
